I consent and agree to allow Minmed Group Pte Ltd (hereinafter referred to as Minmed), all its related subsidiaries and partner laboratories Quest Laboratories Pte Ltd (hereinafter referred to as Quest) to electronically transmit my health screening data on an individual and aggregated format from the onsite screening session held on 27th April 2018, 30th April and 2 May 2018 at AIA office located at 3 Tampines Grande Singapore 528799, 1 Robinson Road, AIA Tower Singapore 048542 and 371 Alexandra Road Singapore 159963 respectively, and offsite health screening during the extension at Minmed Health Screeners and the following 7 clinics (mainly Minmed Medical Clinic at Sengkang, Haig Road, Punggol, Yishun and Jurong East, Dayspring Medical Clinic at Pasir Ris and Tampines) from 27th April 2018 to 31st May 2018, to MyDoc Pte Ltd of 136B Telok Ayer Street Singapore 068601 (herein referred to as MYDOC) and/or AIA Singapore (herein referred to as AIASG) in a secured data transmission mode.

I acknowledge that the transmission of health screening data to MYDOC is for the sole purpose of report retrieval for this exercise only and/or the transmission of health screening data to AIASG is for AIA Vitality programme.

Pursuant to the Personal Data Protection Act 2012 (Act 26 of 2012), any personal data provided to Minmed Group Pte Ltd will be used for the purpose of programme participants' identification, mailing of personal health data, related communication & follow-up for the agreed health and wellness initiative or programme. Minmed Group Pte Ltd shall take all reasonable precaution to protect & preserve the integrity of personal data provided for the said purpose.
